Mounted models of originals, to be used as reference for surface morphology.

Detailed instructions for shading, photos with shade tabs, and photos of preparations, with stump shades

Bite of anterior preparations in MI if necessary

Full arch impression of preparations with a custom tray

New impression of approved upper provisional preparations

Facebow record of approved provisional restorations, (not of the preparations). It is best to face bow mount the upper model in the dental office, this avoids possible distortion during shipping.

New impression of opposing arch.

Complete series of photographs of provisional restorations without retractors.

List specific changes to be made from provisional restorations (egg, incisal edge position, shapes, length, midline, etc.), also list the ideal length of finished restorations, (we should get this from the new model of the approved provisional restorations)

Detailed written instructions with description of case clearly marked on prescription.
